This 60 minute Telehealth or In person consultation incudes a detailed assessment of your medical history, lab orders, stress and sleep evaluation. We will also go over any current desires for weight loss assistance, nutritional assessment and fitness routine. A follow up lab discussion will need to be made 7-14 days after the initial evaluation to go over lab results and develop a treatment plan. This is for patients using their Health Insurance Benefits. Insurance Verification is required to schedule. Co-pays or Deductibles will be collected prior to the visit with the provider.
